Information for Funeral Directors
Funeral directors can register certified death certificates at Columbus Public Health, 240 Parsons Ave., Columbus, Ohio 43215 in the Vital Statistics Office on the first floor.
Office Hours:
Monday, Tuesday, Thursday, and Friday 8-12 and 1-4 PM
Wednesday 9-12 and 1-4 PM
Please allow for a 24-hour turn around time for any combination of orders exceeding 50 copies.
Burial Permits are available to funeral directors 24 hours a day, 7 days a week at the following locations:
- Vital Statistics Office (during regular business hours)
- Sub registrar or local Vital Statistics Registrar (it is recommended you contact the appropriate party first for availability prior to approval)
