Communicable Disease Surveillance and Control
Communicable Disease Surveillance and Control Program ensures the proper investigation and follow-up of all reported cases of communicable disease, conducts disease surveillance, implements prevention strategy, educates residents, and manages Perinatal Hepatitis B cases.
Communicable Disease Reporting System (CDRS)
CDRS is a 24-hour phone line for reporting to Columbus Public Health or Franklin County Public Health. This is for health care practitioners and laboratories only. For more information, go to CDRS.
Ohio Department of Job & Family Services requires all licensed child care centers and Type A Homes to have at least one person on site at the center who has completed the 6-hour with Communicable Disease training.

A foodborne illness questionnaire (coming soon to the website) can be completed and FAXed to (614) 645-1620.
